Incident Reporting

All incidents which actually cause or are judged to have the potential to cause harm to individuals or damage to equipment must be recorded.

The Incident Report Form form should be used to record any incidents, accidents or “near misses”.  A separate form should be filled in by all those involved as soon as practicable after the incident has occurred. Paper forms and an Incident Folder are also kept in the boat shed. The EACRC Safety Officer should also be contacted:
